Vitiligo is a long-term skin condition in which the skin slowly loses its colour in patches. The colour in our skin comes from cells called melanocytes. In vitiligo, these cells are damaged or stop working in certain areas, and the skin there turns pale or white.
Doctors believe that in most people, vitiligo is an autoimmune condition, which means the body’s own defence system mistakenly targets these colour-producing cells. Genes play a part, and it often runs in families, though many people with vitiligo have no relatives who are affected.
A few things are worth saying clearly, because they cause a lot of unnecessary worry.
Vitiligo can start at any age, and affects men and women equally. It usually does not cause pain or physical discomfort, but its effect on how people feel about themselves can be real. Support for that is part of good care, and there are treatments that can help.
Vitiligo patterns vary. Some people have just a few patches that stay the same for years. Others see them spread over months. In a smaller group, the patches are limited to one side or area of the body, which is called segmental vitiligo and tends to behave differently from the more common type.
The exact cause of vitiligo is not fully understood. Most researchers think several factors combine in a person who is already prone to it.
The immune system attacks the pigment-making cells in the skin. This is the leading explanation for the most common type of vitiligo.
Having a close relative with vitiligo or another autoimmune condition raises the chance of developing it, though most children of people with vitiligo do not develop it.
Vitiligo is more often seen in people with thyroid disease, and sometimes with other conditions such as type 1 diabetes, alopecia areata, or pernicious anaemia. This is why a doctor may suggest a few blood tests.
Skin injury, severe sunburn, significant emotional stress, or chemical exposure may precede the first patches in some people. They do not cause vitiligo on their own, but may bring it out in someone who is prone to it.
Pityriasis alba, fungal infections, post-inflammatory pale patches after eczema or injury, and leprosy can all resemble vitiligo. Treating the wrong condition wastes time, which is why the diagnosis should be confirmed first.
The first step is a conversation. Your doctor will ask when the patches began, where they appeared, whether they have grown or changed, whether you have had a skin injury or sunburn in the area, and whether anyone in your family has vitiligo, thyroid disease, or other autoimmune conditions. Treatments you have already tried are useful to know about.
The doctor will then examine your skin carefully, often using a special lamp (Wood’s lamp) that makes pigment loss easier to see, and sometimes a magnifying device called a dermatoscope. In most cases, this is enough to confirm the diagnosis.
Depending on your situation, your doctor may suggest:
Such as a thyroid profile, and others when clinically indicated
Only if the diagnosis is unclear
With photographs, to track whether patches are changing
When white patches appear, many people try whatever they hear about. Some turn to herbal pastes or home remedies passed on by relatives, some apply strong creams from the chemist, and some simply wait and hope.
Several conditions look like vitiligo and are treated differently.
Treatment choices and the chances of a good response depend partly on whether the patches are stable or still spreading.
In many cases, treatment tends to give better results when it is started earlier, though responses vary from person to person.
Some unproven pastes and applications can cause burns, blisters, or allergic reactions, and may leave behind new marks.
A simple thyroid test, for example, can pick up a problem that is easily treated.
Support for the emotional side of vitiligo is part of care, and it matters.
Vitiligo is not an emergency, but certain situations call for an early visit.
See a doctor the same day if you develop a painful, blistering sunburn on affected skin, or if you notice signs of infection after a skin injury, such as increasing redness, swelling, warmth, or pus.

Care planned around your skin, at a pace that suits you.
Treatment depends on how much skin is involved, where the patches are, whether the condition is active, your age, and your preferences. A plan may include some of the following.
It is important to be realistic. Vitiligo is a long-term condition. Treatment can help stop the spread and bring colour back in many people, but response varies a great deal from person to person and from one body area to another. Some areas, such as the face, tend to respond better than others, such as the hands and feet. Treatment often takes months, and results cannot be guaranteed. Patches can also return after treatment ends.
Please avoid applying herbal pastes, plant extracts, or strong creams to the patches without medical advice. Some can cause burns and blisters, especially when combined with sunlight.
Such as topical corticosteroids or calcineurin inhibitors, prescribed for the right area, strength, and duration
Which uses controlled doses of ultraviolet light under medical supervision, and is commonly used for more widespread vitiligo
In selected cases, to help control spreading when the condition is active
Which your doctor may discuss if they are suitable for you
Such as skin or cell grafting, which may be considered for stable patches that have not responded to other treatments, and which suit only certain patients
Using sunscreen and cover-up measures, since affected skin burns easily and sun exposure can make the contrast more noticeable
Such as skin-matched cosmetics, for those who want them
Such as thyroid disease, where present
Which is a real part of treatment, not an extra
To see how your skin is responding and adjust the plan
No. Vitiligo cannot spread from one person to another through touch, shared food, clothing, or living together.
It is thought to be mainly an autoimmune condition, where the immune system damages the pigment-making cells of the skin. Genetics play a role, and in some people triggers such as skin injury or stress may come before the first patches. It is not caused by diet, and it is not caused by anything you did wrong.
There is currently no guaranteed cure. However, treatment can help stop the spread and bring back colour in many people. How well it works differs from person to person, and patches can come back, so your doctor will give you a realistic picture of what to expect.
Usually months rather than weeks. Many treatments need to be used consistently for several months before a change is visible, and some patients need longer treatment. Your doctor will review progress with you at regular intervals.
This is hard to predict. Some people have patches that stay the same for years, while in others they spread, sometimes in phases. Your doctor can assess whether yours seem active and what that means for treatment.
There is no proven link between particular foods and vitiligo. A balanced, nutritious diet is good for general health, but you do not need to follow strict food restrictions because of vitiligo. Your doctor can advise you if any deficiency or other condition is found.
The affected skin has little natural protection and can burn easily. Using a broad-spectrum sunscreen and covering up when in strong sun is generally recommended. Ask your doctor about the right sun protection for you.
Yes, vitiligo can start in childhood. Treatment is planned carefully for children, with attention to the strength and duration of medicines and the emotional effect on the child. A doctor should guide treatment in children.
