Most fevers settle within a few days. A chronic, prolonged, or recurrent fever lingers well beyond what an ordinary infection would explain, or disappears and returns again and again. It may be high, or a mild temperature that creeps up in the evening.
As a rough guide, a fever that has not improved after about a week, or keeps returning over several weeks, should be assessed. Causes include tuberculosis, typhoid, urinary infections, hidden abscesses, autoimmune conditions, medicines, thyroid problems, and, less often, cancers.
“What does not work is guessing. Repeated antibiotics for a fever of unknown cause can hide clues and make the real problem harder to find.”
Some people have none of these, and simply have a fever that will not go away. Both deserve a careful look.
Fever that returns, often in the evening or at night.
Drenching night sweats, or chills that come and go.
Unexplained weight loss, or loss of appetite.
Ongoing tiredness and weakness.
A cough that will not settle, or breathlessness.
In the neck, armpit, or groin.
A doctor cannot tell the cause of a long-lasting fever from symptoms alone. These are the main groups considered.
One of the commonest causes in India, and it can affect organs beyond the lungs.
Typhoid, malaria, scrub typhus, brucellosis, and other infections.
Urinary infection, heart valve infection, or a collection of pus.
Lupus, rheumatoid arthritis, or vasculitis causing ongoing inflammation.
Some medicines can cause fever. Do not stop them without advice.
Thyroid problems, blood clots, or bowel inflammation.
Lymphoma or leukaemia, with fever, weight loss, and night sweats.
We keep monitoring and testing, because the picture often clears with time.
Your story often holds the biggest clue. We listen carefully, examine thoroughly, and test in a logical order.
Fever pattern, travel, contacts, animals, raw milk, medicines, and antibiotics already taken.
Checking glands, heart, liver, spleen, skin, chest, and joints.
Blood and urine tests, cultures, infection tests, and imaging, in a logical order.
The right treatment for the cause, until the fever settles and reports are normal.
We listen first, explain what we think is happening in language you can follow, and only suggest tests and treatments that make sense for you.
You will not be hurried. Many patients say no one had asked them the questions we did.
What each test is for, and what the reports mean, without heavy jargon.
Tests chosen from your story and examination, in order, not everything at once.
Your age, health conditions, pregnancy, and daily life shape the plan.
If the cause is not clear yet, we will tell you, and explain what we do next.
We review you until the fever settles and the cause is fully addressed.
A clear, structured path from your first consultation to the care that is right for you.
Your story often holds the biggest clue.
Looking carefully for signs that point to the cause.
Blood cultures ideally before any antibiotic is started.
What each test is for, and what it shows.
The right medicine for the right germ, for the right length of time.
Medicines that calm the immune system, with regular monitoring.
Fluids, rest, and specialist input when needed.
Confirming the fever has settled and reports are back to normal.
A fever that keeps returning deserves answers. At DR Care, our team is here to listen, investigate carefully, and treat what is really behind it.

Antibiotics or steroids without a diagnosis can mask the cause and delay the right treatment.
People with a fever that has lasted more than a week, keeps coming back, or comes with weight loss, night sweats, or a cough can consult DR Care for a careful evaluation.
Without clear improvement.
Settling, then coming back again and again.
A temperature that creeps up later in the day.
Or drenching night sweats and poor appetite.
Or breathlessness or swollen glands.
Especially to areas with malaria or other infections.
Fever returns every time the course ends.
Diabetes, heart, kidney, or liver disease, or pregnancy.
There is no single answer for everyone. As a general guide, a fever that has not improved after about a week, or that keeps returning over several weeks, should be assessed, and sooner if there are warning signs, or if the person is a baby, an older adult, pregnant, or has a health condition.
A mild temperature rise, especially in the evening, is easy to dismiss. But if it persists for weeks, particularly with weight loss, night sweats, or tiredness, it should be checked.
Yes. TB is one of the commonest causes of prolonged fever in India, and it can affect organs other than the lungs. That is why TB testing is often part of the evaluation.
Antibiotics work only against bacteria, and only if the right one is used for long enough. If the cause is something else, such as tuberculosis, a virus, or an autoimmune condition, the fever returns once the antibiotics stop. Repeated antibiotics without a diagnosis can also make the cause harder to find.
Not always. Many people can be assessed and treated as outpatients. Admission is advised if you are very unwell, dehydrated, have warning signs, or need tests and monitoring that are best done in hospital.
This can happen, and it does not mean your fever is not real. Your doctor will keep monitoring you, repeat selected tests, and re-examine you. The cause often becomes clear with time.
Paracetamol can bring comfort, but taking it repeatedly without finding the cause only hides the problem. Take it at the dose your doctor advises, and do not use it to avoid getting checked.
Go straight away if fever comes with confusion or extreme drowsiness, difficulty breathing, a severe headache with a stiff neck, a seizure, bleeding or a rash like small bruises, repeated vomiting with signs of dehydration, or cold, clammy skin. Seek urgent care for any fever in a baby under three months, or in someone on chemotherapy.
