Frozen shoulder (adhesive capsulitis) happens when the capsule around the shoulder joint becomes inflamed, thick, and tight. The joint loses room to move. The hallmark is that you cannot move the shoulder fully, and neither can anyone else when they try to move it for you.
It often passes through stages: a painful phase, a stiff phase, and a thawing phase as movement slowly returns. It is most common between 40 and 60, and more common with diabetes, thyroid problems, or after keeping the shoulder still.
“Most people recover much of their movement, but it is not instant. Doing nothing can mean a longer, more painful course, and early treatment often makes the journey more comfortable.”
Stiffness usually builds over weeks. If any of these feel familiar, it may be time to get your shoulder checked.
A dull ache in the shoulder, sometimes spreading down the upper arm.
Pain that disturbs sleep, especially lying on that side.
Hard to reach behind your back, put on a shirt, or fasten a bra.
Difficulty raising the arm to the side or in front, or turning it out.
Stiffness that gradually increases over weeks.
Sharp pain when you try to move beyond a limited range.
Often there is no clear trigger. These are the factors linked with a higher chance, and the conditions that can look similar.
In many people, it develops without any obvious trigger.
More likely with diabetes, and may be slower to settle.
Thyroid problems and some other conditions raise the chance.
Keeping the shoulder still after injury, fracture, surgery, or stroke.
Most common between about 40 and 60.
A little more common in women.
Tears, tendinitis, arthritis, or calcium deposits can look similar.
Pain from the neck can mimic shoulder trouble.
Treatment differs at each stage, so we confirm the diagnosis and plan care that eases pain and helps movement return.

Keeping the shoulder still can make a frozen shoulder worse. Guided movement helps.
People with shoulder pain, stiffness, night pain, or trouble lifting the arm can consult DR Care for a proper evaluation and a stage-matched plan.
Pain that wakes you or stops you lying on that side.
Struggling with shirts, buttons, or reaching behind you.
Unable to comb your hair or reach up.
A stiffening shoulder alongside diabetes.
Shoulder stiffness with a thyroid problem.
Stiffness after a fracture, surgery, or stroke.
The age group most often affected.
Balms and painkillers no longer working.
It varies a lot. It often improves gradually over many months and can sometimes take longer. Treatment aims to ease pain and help movement return more comfortably.
Many cases improve with time, but it can be slow and painful. Proper treatment often makes the course easier.
Rest alone tends to make it stiffer. Gentle movement and guided exercises usually help. Avoid forcing painful movements.
People with diabetes are more likely to develop frozen shoulder and may recover more slowly. Good sugar control helps overall.
Not always. It is mainly diagnosed by examination. Scans are used if another cause such as a rotator cuff tear is suspected.
They are often used and can reduce pain, but they are not for everyone. If you have diabetes, they can temporarily raise your blood sugar. Your doctor will discuss this with you.
Most people do not. Procedures are considered only when there is severe stiffness despite many months of proper treatment.
Go straight away if shoulder or arm pain comes with chest pressure, sweating, or breathlessness, with sudden one-sided weakness, a drooping face, or trouble speaking, or if the shoulder is hot, red, and swollen with fever.
