ALS (amyotrophic lateral sclerosis), called SLA in Italian and motor neurone disease in many countries, is a condition in which the nerve cells that control voluntary muscles gradually stop working. As messages stop getting through, muscles become weaker and thinner over time.
It affects strength, movement, speech, swallowing, and eventually breathing. Sensation is usually spared, and thinking and memory remain normal in most people. Many other conditions can look like ALS, and several of them are treatable.
“ALS cannot currently be cured, but it can be managed. Care focuses on slowing progression where possible, relieving symptoms, and keeping people comfortable, independent, and well supported.”
ALS often begins in one area and gradually spreads. Twitching and cramps alone are usually harmless; ALS is suspected when they come with progressive weakness.
Clumsiness, a weak grip, dropping things, or trouble with buttons.
A dragging foot, or difficulty climbing stairs or rising from a chair.
Visible wasting, twitching under the skin, cramps, or stiffness.
Slow or slurred speech, or a change in voice.
Choking on food or liquids, or drooling.
Later on, especially when lying flat, with poor sleep and morning headaches.
The exact cause of ALS is not fully understood. Because treatable conditions can look similar, doctors rule them out before reaching a diagnosis.
Most often diagnosed in middle age and later, though it can occur earlier.
A smaller number have an inherited form; genetic counselling may be offered.
No single cause is proven for most people. Nothing you did is to blame.
Pressure on the spinal cord or nerve roots in the neck or lower back.
Vitamin B12 deficiency, thyroid disease, and metabolic problems.
Myasthenia gravis, neuropathies, or inflammatory muscle disease.
Some infections, including HIV, and medicine side effects.
Especially when speech and swallowing are affected.
We look for treatable causes first, and if it is ALS, coordinate care that supports comfort, safety, and independence.
When symptoms began, how they changed, and how speech, swallowing, breathing, and daily life are affected.

Family members are welcome at every visit, and we answer their questions too.
People with progressive weakness, muscle thinning with twitching, slurred speech, or swallowing difficulty, and those already diagnosed, can consult DR Care.
Weakness that is getting worse over time.
A foot that catches or drags.
Muscle wasting along with twitching or cramps.
Slurred speech or choking that is not improving.
Unexplained weight loss alongside weakness.
Wanting assessment or genetic counselling.
Needing a coordinated care plan.
Wanting a careful, independent review.
Yes. SLA is the Italian abbreviation for Sclerosi Laterale Amiotrofica, which is the same condition as amyotrophic lateral sclerosis (ALS), also called motor neurone disease (MND) in many countries. In some parts of the world, it is also called Lou Gehrig’s disease.
Usually not. Twitching and cramps are very common and are most often harmless. ALS is considered when twitching comes with muscle weakness and wasting that are getting worse. If you are worried, an examination can tell you a lot.
There is no single test. A neurologist makes the diagnosis by examining you and using tests such as EMG and nerve conduction studies, MRI scans, and blood tests, mainly to rule out other conditions. It may take time, and sometimes repeat examinations are needed to be sure.
There is currently no cure. However, some medicines may slow progression a little in certain people, and good supportive care can help with symptoms, safety, and quality of life. Research is active, and your doctor can tell you about developments that may apply to you.
Care usually combines medicines, physiotherapy, occupational therapy, speech and swallowing therapy, nutrition support, breathing support when needed, emotional support, and palliative care. Different people need different things at different times, and your plan is adjusted as your needs change.
Most people with ALS have no family history. A smaller proportion have an inherited form. If someone in your family has ALS, you can speak to your doctor about genetic counselling and whether testing is appropriate.
In most people, thinking and memory stay normal. In some, there can be changes in behaviour, planning, or language, and these can be supported. Please tell your doctor if you or your family notice changes.
Go straight away if there is sudden weakness on one side with a drooping face or difficulty speaking, severe breathlessness, choking that does not clear or an inability to swallow saliva with fever, blue lips, confusion or extreme drowsiness, or a fall with injury.
