Almost everyone has back pain at some point. It is one of the most common reasons people visit a doctor and one of the most common reasons they miss work.
Your spine is a stack of bones (vertebrae), cushioned by soft discs, held together by ligaments, and supported by layers of muscle, with the nerves of the spinal cord running through the middle. That is a lot of moving parts, and pain can arise from any of them.
The good news is that most back pain is not caused by anything dangerous. A large share of it comes from strained muscles and ligaments, or from stiffness in the joints of the spine, and it tends to improve over a few weeks with the right care. But some back pain comes from a slipped or bulging disc pressing on a nerve, from wear and tear in the spine, or occasionally from a condition that needs prompt attention.
The difficulty is that the type of pain alone does not tell you which one you have. That is why back pain that keeps returning, or does not settle, deserves a proper look instead of another round of painkillers and balms.
Back pain can feel different from person to person. You might notice:
Notice what makes it better and worse. Pain that eases when you lie down and is worse when you bend may suggest something different from pain that eases when you bend forward. These details help your doctor.
It is not possible to tell what is causing back pain by symptoms alone. These are the main possibilities doctors consider.
The most common cause. It can follow lifting something heavy, sudden twisting, an awkward posture, or simply overuse.
Long hours at a desk, driving, or looking at a phone, combined with weak core and back muscles, put steady strain on the spine.
The discs between the vertebrae can bulge or slip (a prolapsed or herniated disc), sometimes pressing on a nerve and causing pain, tingling, or weakness that travels down the leg or arm.
Over time, the discs and joints of the spine can wear down. This is very common with age and does not always cause symptoms, but it can lead to stiffness and pain.
The space around the nerves narrows, often causing leg pain or heaviness when walking that eases with rest or sitting.
The joints at the base of the spine or between vertebrae can become irritated or inflamed.
Conditions such as ankylosing spondylitis can cause back pain and morning stiffness that eases with movement, usually in younger people.
Osteoporosis can lead to small compression fractures in the spine, sometimes with little or no injury.
These are uncommon but serious causes, and are one reason a proper examination matters.
Kidney stones, infections, gynaecological problems, and some abdominal conditions can cause pain felt in the back.
Excess body weight, smoking, weak core muscles, lack of exercise, poor sleep, and stress can all make back pain more likely or more persistent.
It starts with a conversation. Your doctor will ask when the pain began, where it is, whether it spreads, what makes it better or worse, whether there was an injury, and how it affects your sleep, work, and daily activities. Your job, posture, exercise habits, previous back problems, and general health matter too.
The doctor will then examine your back, checking your posture, how you move, areas of tenderness, and muscle strength. A neurological examination of the legs, including reflexes, sensation, and strength, helps find out whether a nerve is affected.
Depending on what is found, your doctor may suggest further tests, such as:
To look at the bones and alignment of the spine
If nerve involvement is suspected, if there are warning signs, if pain has not improved with treatment, or if procedures or surgery are being considered
If inflammation, infection, or other conditions need to be checked
If osteoporosis is a concern
Not everyone needs every test, and many people with ordinary back pain do not need a scan at all. Your doctor will explain which ones are useful for you, and why.
When the back hurts, the first instinct is to take a painkiller, apply a balm, or rest in bed until it passes. For a minor strain, that can be reasonable for a short while. But when pain keeps returning, or comes with other symptoms, treating the pain alone does not deal with the reason behind it.
A muscle strain, a slipped disc, and a narrowed spinal canal need different treatment.
Nerve compression, infection, fractures, and other conditions are best treated promptly.
Long bed rest can actually slow recovery. And the wrong exercise can worsen some conditions.
Many people with ordinary back pain do not need an MRI, and scans can show age-related changes that are not the cause of the pain.
You learn what to do, what to avoid, how to move safely, and what to watch for.
Most back pain is not an emergency. But go to the nearest emergency department straight away if back pain comes with any of the following:
What we can promise is how we approach every visit. We listen first, we explain what we think is happening in language you can follow, and we only suggest tests and treatments that make sense for you.
You will not be hurried through your history or your questions.
We explain what is likely behind your pain and what your scan or report does and does not mean, without heavy medical jargon.
Your age, work, daily routine, activity goals, and other health conditions all shape the plan.
We will explain non-surgical and surgical choices plainly, and we will not recommend a scan or procedure you do not need.
If pain continues or changes, you can come back and the plan can be reviewed.
Speak to a doctor who will listen, examine, and help you plan the next steps.

Care planned around your daily life, at a pace that suits you.
Treatment depends on the cause, how severe the pain is, how long you have had it, and your overall health. Most people improve without surgery. A plan may include some of the following.
Please avoid regular long-term use of painkillers without medical advice. Frequent use of anti-inflammatory tablets can affect the stomach and kidneys.
Back pain often improves in a few weeks, but it can come back. The aim of treatment is to relieve pain, restore movement, and reduce the chances of it returning. Responses differ from person to person, and results cannot be guaranteed.
Gentle movement and walking usually help recovery more than prolonged bed rest.
Using medicines chosen carefully for your health, along with heat application where it helps. Some medicines, such as anti-inflammatory tablets, are not suitable for everyone, particularly people with stomach, kidney, or heart conditions.
In selected cases, for a limited period
To build core and back strength, improve flexibility, and correct movement patterns
Covering your workstation, sitting habits, lifting technique, and sleeping position
Where relevant, to reduce load on the spine
In selected cases, such as targeted injections near an irritated nerve or joint, after discussing the potential benefits, limits, and risks
Such as medicines for inflammatory conditions, treatment for infection, or bone-strengthening treatment for osteoporosis
When it is needed. This is considered for conditions such as nerve compression with significant or worsening weakness, loss of bladder or bowel control, or severe pain that has not improved with proper non-surgical treatment. Your doctor will explain the options, benefits, and risks so you can make an informed decision.
To check progress and adjust the plan
Many episodes of ordinary back pain improve within a few weeks. Some take longer, and some come and go. If pain lasts beyond a couple of weeks, is getting worse, or comes with leg symptoms, it is worth seeing a doctor.
A short period of rest in the first day or two may feel necessary, but prolonged bed rest usually slows recovery. Gentle movement and returning to normal activity as comfort allows tends to help. Your doctor will guide you if you have a specific condition.
Not always. Most people with ordinary back pain do not need one. An MRI is helpful when there are warning signs, when nerve involvement is suspected, when pain has not improved with treatment, or when surgery or injections are being considered. Scans can also show age-related changes that are common in people without pain, so the results need to be read alongside your symptoms.
Sciatica is pain that travels from the lower back or buttock down the leg, often caused by irritation or pressure on a nerve root, for example from a disc problem. It can come with tingling or numbness. A doctor can help find the cause and suggest treatment.
No. Most people with a slipped disc improve with non-surgical treatment over weeks to months. Surgery is usually reserved for cases with significant or worsening weakness, loss of bladder or bowel control, or severe pain that has not improved with proper treatment.
Prolonged sitting, especially with poor posture and little movement, is a common contributor. Regular breaks, a supportive chair, a well-arranged workstation, and exercise to strengthen the core and back can help.
The right exercise usually helps. But the type and amount matter, and some exercises are not suitable for certain conditions. A doctor or physiotherapist can advise which movements are safe and useful for you.
Go straight away if back pain comes with loss of control over your bladder or bowels, numbness around the groin or buttocks, weakness in both legs or weakness that is quickly getting worse, fever with severe pain, or if it follows a significant fall or accident.
